I keep hearing a card is off center. Are they talking about the pokemon not being in the middle of the picture?

Usually they mean the printed design sits unevenly within the card's cut edges, so one border looks wider than the opposite one. It isn't about where the artist placed the Pokémon. I look at the front and back and keep the grading company's standards in mind. For a personal binder, you may decide a slightly uneven border doesn't bother you at all.
